Housing Support Teams

The NC Division of Mental Health, Developmental Disabilities and Substance Abuse Services put out an RFA for the Homeless Mental Health Housing Initiative. In preparing to write a, hopefully, successful proposal I am doing a little research on what models exist.
East Lindsey District Council "Housing Support Team's aim is to empower people by offering support, advice and guidance to manage their housing related issues and also to realise and achieve their personal goals.
The Housing Support Team work in imaginative ways to address the effects of homelessness and poor housing on individuals or families. Homelessness can be caused by lack of affordable housing or by people's difficulty in making and keeping a home."- The Alleghany County "Housing as Home" plan. Not clear on where this group is located.
